The Advanced
Clerkship in Medicine is a one-month rotation on the inpatient
medicine service which provides 4th-year medical students additional
training and experience in managing complex medical issues. Students often
elect the Advanced Clerkship to better prepare for an internship year in
Medicine, to make a decision about internal medicine residency, or as a last
opportunity to study Internal Medicine and the approach to care of the
complex patient prior to residency training in another field. In fact, the
Department of Medicine encourages all students to take a Medicine
Subinternship prior to graduation, as it is an outstanding preparation for
any post-graduate training.
The Department of Medicine is committed to providing students with
opportunities beyond the Basic
Inpatient Experience, and so has structured the Advanced Clerkship in
Medicine to approximate the role of
the intern. Typically two subinterns (advanced clerkship students) will take the place of one intern on a ward
team, assuming almost all of the care responsibilities for a fewer number of
patients, but engaged in similar activities of an intern: taking overnight
call, admitting patients, entering orders, performing ward procedures, etc.
Pre-requisite:
Successful completion of the
Basic Medicine Inpatient Clerkship
is required for all of the subinternships offered by the Department
of Medicine.
HOW To elect the Advanced Clerkship in
Medicine on the osler, CAROL JOHNS, MEG, or polk SERVICES:

how to elect a Sub-I spot. See below for descriptions.
At Johns Hopkins Hospital, we offer thE 4
FOLLOWING Ward Experiences DESCRIBED BELOW, which are used for the Advanced
Clerkship:
1.
The Osler Firms
(Barker, Janeway, Longcope, & Thayer),
each of which can accommodate 2 clerks, during the following months of the
year only: April, May, June, August, September, and October. The team will
consist of the Assistant Chief of Service, a Firm Faculty Teaching
Attending, two Senior Residents, three Interns, two Subinterns, and three
Basic Medicine Clerks (except for the
month of August). We do not
accept preference requests for specific Osler Services. If you have any further questions, please contact the Course Director,

Carol Johns Service.
This is a private/managed care service which is staffed by Osler Housestaff
and a General Internal Medicine teaching attending from the Medical Services
Organization. Since the teaching attending is the attending of record,
there are daily work and teaching rounds with a faculty member and the team.
The breadth of patient problems is quite diverse on this service, and
housestaff have appreciated working with faculty knowledgeable in health
care systems approach to care. The
Carol Johns Service is offered during the
following months of the year only: April, May, June, August, September, and
October. If you

Polk Unit.
This is the
inpatient AIDS service, which has been a model for the hospital for team
care of complex patients. Attending faculty on this service are from the
Division of Infectious Disease; in addition, clerks will be working with the
I.D. fellow, 2ND
YEAR RESIDENTS, and a full multidisciplinary team on a daily
basis. Because of the complexity of illness on this service, the service
has requested that
only fourth-year students be assigned.
If you have a particular interest in HIV disease, please state your
preference for this service. If you

The Polk Service is offered yearly pending availability for fourth-year
student only.
HOSPITALIST PROGRAM
SUBINTERNSHIPS:
Hospitalist Program. This is a new Medicine Subinternship on the new
Hospitalist Program. Students will work directly with the Faculty
Attendings (no Interns or Residents) on this general medicine inpatient
service, which admits a mixture of both community patients with "bread and
butter" diagnoses and tertiary referral transfers with more varied uncommon
diagnosis (2 -3 slots each month, 4 week duration). There
is great hands on experience as well as excellent teaching. *Students

ASSIGNMENT SCHEDULE:
The Medicine Sub-I elective is scheduled on a MONTHLY
BASIS however we follow the academic/quarterly calendar dates. Dates
are listed on the "Assignments" page of this website. If you have any

bashar@jhmi.edu. FYI: There will be NO WAITLIST offered. Please see "DROP
POLICY" described below.
We recommend the
following:
If
you have decided that you will be applying for a residency in Internal
Medicine, try to take the
subinternship before September 2009. This allows an early opportunity to identify potential
letter-writers and it allows your undecided classmates more openings later in
Spring/Summer.
If
you have an interest in further clinical experience with AIDS, you will be
given preference for the Polk Unit
rotation; please note this with your preferences. If you have any questions

CREDENTIALING:
Eight weeks prior
to the start of your elective (or sooner), you will be required to complete
credentialing paperwork. See
"Assignments" page on this website for more instructions for
required paperwork.
PROVIDER ORDER ENTRY (POE) TRAINING:
It is
the student's responsibility to schedule Provider Order Entry (POE) Training
BEFORE the start of the Sub-I elective. If you have already
completed POE training during your Basic Medicine Clerkship rotation you
WILL NOT need to return for POE again. However, if you wish to
take a refresher course or if you have any specific POE questions, please
contact POE immediately at 4-0958. POE Training is offered on the
first Tuesday and the last Tuesday of each month. To schedule your POE
Training Session, call 4-0958.
DROP Policy:
DROP
POLICY: Because
of the unique role of the Sub-intern on the Ward Team, during the months of
April, May, and June there is a NO DROP POLICY. You will not
have the option to drop this elective unless you find a replacement.
The Department of Medicine considers the Sub-I assignment to be a contract
with the student to complete the subinternship as scheduled. If you feel
that you will not be able to fulfill your commitment, DO NOT sign up
for the Sub-I elective during these months and allow the slot to be assigned
to a colleague.
FYI:
It is the
student’s responsibility to find a replacement.
There is no waitlist offered.
During other months, you
are required to contact the Course Director, Dr. Bimal Ashar, via email at
bashar@jhmi.edu to obtain permission for any drops.
However, you
must notify Dr. Ashar at least 8 weeks in advance if you are unable
to fulfill your commitment to your assigned Sub-I elective. You will
be required to pickup a "DROP ELECTIVE FORM" from the Registrar's
Office and have it signed by Dr. Ashar.
visiting students:
Due to the complexity of Hospital
Information Systems and the Credentialing Process visiting students will not
be accepted into the Subinternship on the Osler, Nelson, or Polk services.
Visiting students are encouraged to contact Ms. Emma Sullens for information
regarding other Department of Medicine Consult Electives, including the
Advanced Clerkship (Subinternship) in Medicine on the Hospitalist service.

interested in applying for Consult Electives are required to contact Ms.

preferences for the
